Position Summary
To provide medical services to include provision of medical evaluation and treatment, medical education and pharmacological intervention and education. Provision of these services will be documented according to the regulatory agency governing the type of service provided.

License / Certification- MSN Degree.
- NJ Licensed-RN, Primary Care/Family Practice Clinical Specialist
- Graduate of approved Masters' Degree Program in Nursing with a concentration in Primary Care/Family Practice Studies
- Possess a waiver to provide MAT or be willing to obtain within 60 days of employment
- Two Years' Experience as an Advanced Practice Nurse licensed to practice in NJ
Position requires sitting and performing repetitive movement while using the computer during a portion of the day.
Major Responsibilities / Performance Objectives- Practice under the supervision of one or more collaborating staff physicians. The nature of the collaboration will be documented through a collaborative practice agreement signed by the physician and the advanced practice nurse.
- APN prescribes for a Medication Assisted Treatment for clients with co-occurring chronic health condition and substance use disorders.
- Competently prescribe medication in a manner consistent with Agency protocol.
- Provide ongoing medication monitoring, medication education and counseling.
- Develop treatment plans for acute and chronic diseases
- Assess the health history and ongoing health related issues of all patients and make appropriate referrals.
- Conduct screening evaluations.
- Order and evaluate results of diagnostic tests.
- Maintain competence in understanding the changes in health promotion throughout the aging process
- Educate patients on disease prevention and healthy lifestyle habits. Manage overall patient care regarding lifestyle and development issues, emphasizing preventative care and disease management.
- Provide medical oversight for clients including consultation with other involved physicians and comply with coordination of care policies and procedures as established by CPC's Medication Therapeutics Committee.
- Participate with the team in Treatment Plan development and review.
- Documentation is timely and complies with all regulatory standards.
- Documentation and signature are legible.
- Attend CME, or CEU programs to enhance clinical skills and to maintain advanced practice certification.

The position requires prolonged sitting, some bending and stretching. Good eye‑hand coordination and manual dexterity sufficient to operate standard office equipment is required. Employees must have normal range of hearing and eyesight to record, prepare, and communicate information pertaining to this position. The employee must occasionally lift and/or move objects up to 30 pounds.
