JOB DESCRIPTION
Atlantic Visiting Nurse, part of Atlantic Health System, is seeking a full-time hospice Registered Nurse to work 8 AM‑4 PM in the Morris County area, including weekends and on‑call as required. The RN will provide compassionate care to adult and geriatric patients in their homes, managing a caseload of up to four patients per day.
Compensation: $43.00/hour (salary increases with experience). Mileage reimbursement at $.725. Internal referral bonus of $7,500.

Qualifications – Required- Bachelor’s degree in Nursing (if Associate must complete BSN within five years of start date)
- Master’s degree in Nursing or related field
- At least one year of experience in Nursing or related area
- ACLS certification
- Perform patient assessments, determine hospice eligibility, and develop plans of care with patients and families.
- Maintain visit productivity standards and demonstrate knowledge of insurance reimbursement.
- Complete discharge assessments and documentation, including laptop transfers twice daily and admission assessments within 24 hrs.
- Use age‑appropriate teaching methods, evaluate teaching plans, and document patient education tools.
- Assume responsibility for case management: develop, implement, evaluate, and revise plans of care; prioritize needs; demonstrate knowledge of hospice regulations and reimbursement guidelines; communicate progress with the interdisciplinary team; attend IDT meetings every two weeks.
- Maintain a safe environment: follow infection control measures, demonstrate pharmacology and medication management, reconcile medications, and maintain current orders.
- Attend monthly staff meetings and participate in PI meetings and committees as requested.
- Document case management activities and demonstrate knowledge of pharmacology and safe medication management.
- Participate in Performance Improvement: review records, gather data, attend UR meetings, orient and mentor new employees, and cross‑train to learn new skills.
- Assist with agency growth by orienting new employees and accepting additional assignments.
