Eagle Rock Post Acute Care at Cedar Grove seeks a compassionate LPN to deliver resident-first care in our skilled nursing and post-acute rehab facility. Responsibilities include medication administration, treatments, wound care, monitoring vital signs, assisting with ADLs, and accurate EHR documentation. The LPN will collaborate with RNs, therapists, and physicians on care plans, support safe transitions of care, and communicate with families.
