Nurse Practitioner, In-Home Primary Care

• Provide in-home primary care to high-acuity patients
• Practice medicine with a focus on patient care rather than volume
• Take the time needed to address patients’ needs
• Work Monday through Friday
• Practice within an integrated, team-based, physician-driven model
• Conduct appointments with assistant support in select markets

• Licensed as a NP in the state where the Harmony Cares Medical Group office is located
• Federal DEA License/eligibility
• Active CPR/BLS Certification
• Board Certification
• Valid driver’s license and good driving record
• Directly related experience in home health, geriatrics, hospice, and primary care
• Geriatric training/experience preferred

Demonstrates expertise in providing in-home primary care to high-acuity patients, with a strong focus on patient-centered care and team-based collaboration. Holds necessary licenses and certifications to ensure compliance and quality in home health, geriatrics, and primary care settings.
