About This Role

The Case Manager Registered Nurse position is a field-based role focused on delivering quality healthcare services to members in Union and surrounding counties. Ideal for experienced nurses, this role involves significant travel and requires strong clinical judgment and communication skills.

• Pay: $66,575 - $142,576/year

  New Jersey, NJ
• Schedule:
  
  Full-time, Monday - Friday, 8am-5pm EST
• Travel: 50-75% travel required in Hudson and surrounding counties
• Residency:
  Must reside in Union County

• Conduct face-to-face visits in members' homes using comprehensive assessment tools.
• Evaluate medical needs of members enrolled in Managed Long-Term Services and Supports (MLTSS) and Dual Special Needs Program (D-SNP/FIDE).
• Develop, implement, and promote health service strategies and policies.
• Collaborate with providers and care teams to create and authorize care plans.
• Advocate for members to ensure proper and safe discharge with appropriate services.
• Document accurately and timely in the member's electronic health record.
• Mentor new hires once proficiency is attained in the role.

Education:

• Minimum associate's degree or diploma in Nursing required

Experience:

• 3+ years clinical practice experience in hospital or alternative care settings

Licenses &

Certifications:

• Active and Unrestricted RN license in NJ required
• Successful completion of the NJ Choice certification is a contingency of employment
