We provide innovative and compassionate services to individuals struggling with substance use and co-occurring disorders. We are looking for dedicated, caring and compassionate people to work in our Residential/Outpatient Treatment Facilities and Community Programs.

The Lead Case Manager functions as an integral part of the treatment team and provides primary social service care to a caseload of residential addiction treatment clients in residence.
Responsibilities:- Provide case management for a caseload of residential/ addiction treatment male or female in residence/ including the following tasks:
- Serve as the Lead Trainer for new case managers, clinical and OP, residential life staff, and assist in cross training
- Serve as a liaison between Straight & Narrow, Drug Court/Probation, DCP&P and all other social service agencies, including the scheduling of Case Conferences, onsite visits as necessary.
- Ascertain whether client needs birth certificates, social security cards or an identification card. Determine status of their TANF, food stamps, Medicaid benefits (if applicable).
- Send for ad/or make appointments for client to go for any of the above that are missing.
- Open client’s welfare case in Passaic County and/or county of origin when completing treatment. Obtain any documents necessary to do this. Close client’s welfare case in sending county (if applicable).
- Open client’s Medicaid case immediately upon receipt of all required documents (if applicable).
- Assist in training new case managers in all departments.
- Set up a Case Conference with client’s, DCP&P/referral source within first month of client’s admission.
- Schedule outside appointments for welfare, WIC, social security, Probation/ visits, Section 8 applications, job search, trips to IOP for interviews, etc.
- Close out client’s account at S & N before he leaves, transferring all personal documents to her/him. Fill out form that gives client a checklist of what to do upon discharge and give her a letter to take to her first welfare appointment.
- Close out client’s Passaic County welfare case (if applicable).
- Provide clients on caseload with referrals to other agencies (i.e., health. Psychiatric, vocational rehabilitation and public assistance) for concurrent and continued care as part of case management
- Facilitate group and didactics as scheduled by supervisor i.e., Parenting Classes, court orders, etc
- Assist clients with linkages to Parental Early Intervention Services
- Submit documentation of all groups, didactics and individual client sessions to supervisor daily.
- Participate in community outreach services
- Assist with coverage of clinical responsibilities as assigned by your supervisor
- Maintain social services portion of clients’ charts as per the Division of Mental Health and Addiction Services (DMHAS) and CARF guidelines, ensuring that the file is up- to-date for all documentation.
- Attend regularly scheduled case Management Meetings, MDT meetings for clients on caseload, providing information on each client’s progress, legal situation, social service benefits and housing situation.
- Provide referrals for clients to other agencies (i.e., health, psychiatric, vocational rehabilitation and public assistance) for concurrent and continued social services and care.
- Encourage clients’ attendance and participation in AA/NA/GA.
- Provide crisis intervention as necessary.
- Follow all Electronic Health Records (EHR) policies.
- Provide documentation of progress made towards obtaining the CADC/LCADC certification
- Maintain professionalism, ethical conduct and appropriate boundaries at all times while delivering direct services to clients or representing the agency in accordance with agency standards and substance abuse best practices.
- Other duties as assigned by the administration

Qualifications:Must have a minimum of High school Diploma, preferred BA/BS or MA/MS/MSW degree. Minimum experience of 3 years in the human services field. Enrollment in CADC/LCADC courses, experience in case management and knowledge of addiction and co-occurring preferred. Must be computer literate and possess ability to type and have a valid Driver’s license.
