Job Description
Coming Home administers the County’s
Homeless Management Information System (HMIS)and
Coordinated Entry and Assessment (CEA)process and serves as Vice-Chair of the Middlesex County Continuum of Care (CoC). The CoC is a collaborative network of organizations and individuals committed to preventing homelessness, rapidly rehousing those who experience housing instability, and advancing coordinated community-wide solutions. Coming Home also operates direct service case management programs and a housing development program for persons without homes.
Your position would be as the Case Manager for the Housing Supports Program, a relatively new program whereby Medicaid offers a benefit of housing case management for members of its Managed Care Companies (MCOs).This program, in which Coming Home is a provider, allows us to augment our case management services, reaching more persons in need. This position will provide housing case management for persons eligible for the Housing Support program and for the homeless hotline case management program when needed.
- Master the parameters of the Housing Support Program; develop professional relationships with MCOs’ staff; work with CHM administrative assistant to bill and keep track of all billing and receipts for all services provided.[MOU1]
- Supportive Services-Conductoutreachtoreferredclienttodeveloprelationshipsandbuild rapport and to determine eligibility for the program.
- Developan[MOU2]individualized plan to address social determinants of health and other barriers to stable housing and self-sufficiency.
- Services may include assisting the individual with gathering documentation, increasing income, accessing appropriate mental and physical health services, and any other services that assist an individual to find a home andimprovehis/herqualityof life.
- Apply Middlesex County’s Coordinated Assessment Tool on all new clients in order toconnectthemtoproper interventionandservices.
- Assist client with the establishment of personal/entitlement benefits including (GA, SSI,TANF,SNAP,etc.).Helpclienttoobtainvariousformsofidentificationincludingbirthcertificatesandsocialsecuritycards.
- Assist with applications for supportive and subsidized housing and research potential locations for affordable rental housing. Build relationships with prospective landlords,and identify appropriate permanent housing options.
- Meet with established clients on a regular basis to review, evaluate, and support goalcompletion.
- Collaborate with community substance abuse and mental health providers to ensure seamless referral services. Provide ongoing information, referrals, linkages, andadvocacyforallotheridentifiedneeds.

- Documentation - Completion of all necessary paperwork to track client progress, (dailylogs,HMIS,etc.).Maintaindocumentationstandardsassetforthbytheprogram. Ensureallprogramdataisaccurateandenteredintotheappropriateprogramdocumentation system, i.e. HMIS, internal spreadsheet, etc.
