The RN, in collaboration with the physician, implements and evaluates the plan of care for patients and families. The RN coordinates the care provided by the multidisciplinary team during each visit and assures appropriate follow-up between visits and/or referral to community agencies.
Duties & Responsibilities- Implements and evaluates the plan of care for patients and families, in collaboration with the physician.
- Coordinates the care provided by the multidisciplinary team during each visit.
- Assures appropriate follow-up between visits and/or referral to community agencies when required.
- Provides education to patients, families and significant others, and assists the medical staff as necessary.
- Performs all aspects of patient care.
- Documents patient's response to treatment according to established policies and procedures.
- Communicates in a professional manner, providing pertinent information to other staff members and patients/families, including necessary services required for patient and/or family.
- Performs other related duties as required.
- Current RI Registered Nurse license.
- Ability to apply the nursing process to the care of ambulatory patients.
- Ability to identify and meet the learning needs of patients and their families.
- Ability to communicate effectively and work collaboratively with other disciplines.
- Ability to lift and move patients.
- Organizational, priority setting, problem solving and decision making skills.
- Knowledge of community services, support agencies and government regulations.
- Multilingual skills highly desirable.
- OCN certification preferred.
- Knowledge of current antineoplastic therapy preferred.
- Self - motivation.
- Commitment to continuing education.
- Professional accountability.
