Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering residential, outpatient, inpatient, patient‑centered medical homes, medication‑assisted treatment, and crisis services for individuals experiencing opioid use disorder, homelessness, and mental illness across Arizona, Oklahoma, and the District of Columbia.

Direct Care Specialist is responsible for assisting with the admission and orientation of participants in the program, monitoring patient care in both voluntary and involuntary settings, and coordinating discharges.

• Assist with admission and orientation of program participants.
• Monitor ongoing patient care in facility settings.
• Coordinate patient discharges.

• Certified BHT or EMT.
• Clear 39‑month MVR record presented at interview.
• Level‑one fingerprint clearance card.
• Minimum age 21.
• Behavioral health/substance abuse experience is a plus.
