Full-Time Social Worker
Full-time social worker provides comprehensive social work services to patients and families in a hospital setting, including psychosocial assessments, counseling, resource referrals, and discharge planning. Works collaboratively with interdisciplinary teams to develop individualized care plans and advocates for patients' rights. Maintains accurate documentation and participates in ongoing professional development.
Key Responsibilities:- Provide comprehensive social work services, including assessments, counseling, resource referrals, and discharge planning.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Advocate for patients' rights and coordinate community resources to ensure continuity of care.
- Maintain accurate and thorough documentation in compliance with regulatory standards and hospital policies.
- Participate in ongoing professional development and training opportunities.
- Master's degree in Social Work from an accredited institution.
- Current licensure as a Social Worker in the state of Arizona.
- BLS certification required.
Skills:
- Minimum of 2 years of clinical social work experience preferred.
