Full-time Social Worker
Full-time social worker provides comprehensive psychosocial services to patients and families at a partnered hospital in Payson, AZ. The role involves assessments, counseling, resource referrals, discharge planning, and interdisciplinary collaboration to ensure continuity of care.
Key Responsibilities:- Provide comprehensive social work services, including psychosocial assessments, counseling, resource referrals, and discharge planning.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Advocate for patients' rights and coordinate community resources to ensure continuity of care.
- Maintain accurate and thorough documentation in compliance with regulatory standards and hospital policies.
- Participate in ongoing professional development and training opportunities.
- Master's degree in Social Work from an accredited institution.
- Current licensure as a Social Worker in the state of Arizona.
- BLS certification required.
Skills:
- Minimum of 2 years of clinical social work experience.
