Overview
Summary of Duties:
Under the direction of the Director of Social Services, the Multilingual Behavioral Health & Substance Use Disorder Care Coordinator (Care Coordinator) designs and implements intervention and support programs to assist Peabody residents and their families in accessing behavioral health, medical health, and substance use disorder services. The Care Coordinator works specifically with residents whose primary spoken language is not English.
- Provide care coordination to Peabody residents whose primary language is Spanish, Portuguese, or another non-English language.
- Increase access to inpatient and outpatient behavioral health, medical health, and substance use disorder treatment resources.
- Coordinate or participate in regular substance use and mental healthcare provider meetings within the City of Peabody.
- Develop proactive outreach programs for residents whose primary language is not English.
- Develop proactive outreach programs and supports for residents who reside in public housing.
- Provide on-going community-based support and consultation to individuals and families.
- Connect residents to existing support and education groups and coordinate and facilitate support and education groups as needed.
- Host and participate in community-wide educational and outreach events.
- Conduct follow-up with residents after police response.
- Assist the Mental Health/Substance Abuse Clinician with client assessment and intake.
- Maintain data collection related to the position, including referrals, responses, outcomes, and other relevant metrics.
- Coordinate prevention strategies with the Youth Outreach Coordinator.
- Coordinate treatment and recovery strategies with the Mental Health/Substance Abuse Clinician.
- Supervise interns as needed.
- Perform other duties as assigned or required.
32.5 hours per week – Monday–Wednesday: 8:30 AM – 4:00 PM;
Thursday: 8:30 AM – 7:00 PM;
Friday: 8:30 AM – 12:30 PM. Additional evening and weekend hours may be required.
$60,909 – $64,619 DOQ plus Stipend: 6% of annual salary
