Job Overview
North Shore Community Health (NSCH) is an industry leader in community healthcare delivery. We are seeking a Registered Nurse to provide direct and indirect nursing care under the general direction of the Clinical Nurse Manager. The RN will manage medication refills, provide patient education, triage/assessment over the phone and in person, and collaborate with the triage nurse team, site supervisor, medical director, and other health center team members to maintain a safe, effective work environment.
Cultural competence is required.
- Assess patients’ basic health status and related care needs for patients who call the clinic or who present for scheduled or unscheduled care.
- Analyze and interpret data to make informed judgments related to patient care.
- Evaluate outcomes of basic nursing interventions and initiate changes in care plans.
- Collaborate and communicate with primary care providers and other team members to ensure quality and continuity of care, including participation in huddles to smooth workflow.
- Review refill requests from pharmacy and patients based on medication refill protocol; reconcile medication lists and provide necessary information for provider authorization.
- Administer immunizations and medications and perform procedures as needed (EKGs, suture removals, wound dressing changes, ear irrigations, PPD application and interpretation, blood pressure checks).
- Maintain accurate and timely documentation of all nursing care and patient communication, including phone notes, nursing visit notes, and immunization or medication administration records.
- Complete patient forms to the fullest extent possible within the scope of nursing practice, including prior authorization forms.
- Provide health education to individuals and families to support patient self‑management and empowerment.
- Communicate with members of the patient’s care team outside the health center as needed (e.g., specialists, visiting nurses) to coordinate care.
- Manage vaccination supply and ordering, including proper storage and handling.
- Use data trends for self‑directed learning and performance improvement.
- Provide coverage at other NSCH clinical sites or call centers as needed.
- Perform additional duties to fulfill NSCH's mission as assigned.
- College degree from an accredited school of nursing and current RN license in Massachusetts.
- Current CPR certification.
- Ability to function effectively as a leader and team member, and independently when required.
- Ability to work in a culturally diverse environment.
- Excellent communication and interpersonal skills.
- Problem‑solving mindset with a positive attitude.
- High level of customer service toward patients and coworkers.
- Strong critical thinking skills to prioritize competing demands.
- Quick response to patient or unit condition changes.
- Preference for bilingual/bicultural individuals who can speak Spanish and/or Portuguese.
- Proficiency with electronic health records and strong computer skills.
- Sensitivity in working with patients with complex medical, psychosocial, and socioeconomic circumstances.
- Travel to other NSCH sites for meetings and clinical coverage; evening and Saturday hours required.
