** Home Health Licensed Practical Nurse (LPN)**## PRN | Beverly to Peabody $32–$38 per hour, based on experience

** What You’ll Do
*** Provide skilled nursing care under the direction of an RN and physician
* Follow individualized plans of care and physician orders
* Monitor patients and report changes to the RN Case Manager
* Educate patients and caregivers on medications, treatments, and self-care
* Complete accurate and timely clinical documentation
* Collaborate with the care team to improve outcomes and help prevent avoidable hospital visits## ## #
** What You’ll Need
*** Current Massachusetts LPN license
* Graduate of an accredited nursing program
* At least one year of home care experience
* Valid driver’s license, reliable transportation, and current auto insurance
* Ability to travel throughout Malden, Lowell, and surrounding communities
* Home health or hospice experience preferred
* Ability to sit, stand, bend, lift and move intermittently and lift 50–100 lbs.
