Essential Functions:

The licensed registered professional nurse shall supervise the provision of personal care tasks for clients and for clients determined to be medically frail or medically compromised. The nurse may also supervise companions. Such supervisor may perform other duties provided he or she is able to fulfill the supervisory responsibilities described in these rules.

MINIMUM QUALIFICATIONS:
1. Current unrestricted State Registered Nurse license and CPR certification.
2. Graduate of an accredited nursing program.
3. One year of continuous nursing experience or equivalent.
4. Ability to meet Agency’s bonding criteria. This includes never having been shown by credible evidence to have abused, neglected, sexually assaulted, exploited, or deprived any person or to have subjected any person to serious injury as result of intentional or grossly negligent misconduct as evidenced by the oral or written statement to this effect obtained at the time of application.

Further, never having made any material false statements concerning qualification requirements to Agency or the Department of Human Resources.
5. Valid driver’s license, available personal vehicle, and proof of current auto insurance.
6. Concise verbal, written, and organizational skills.
7. Five years of verifiable employment history, or a complete employment history if employment hasn’t exceeded five years.
8. Administrator of a satisfactory TB screening test upon employment and annually thereafter.

DESIRED QUALIFICATIONS:
Extensive assessment skills. At least two years of nursing experience, and ACLS certification. At least one year of work in home care industry.
PHYSICAL REQUIREMENTS:
  • Demonstrated ability to effectively communicate and interact with families, co-workers, and all supervisors.
  • Including ability to proficiently follow and process written and verbal instructions in the English language.
  • Hearing and visual capacities sufficient for accurate communication with all clients and co-workers, including assessment and recording of clinical information.
  • The ability to lift, turn, and transfer clients in need.
  • GENERAL RESPONSIBILITIES:
    1. Supervise all caregiver staff by making supervisory visits to each client’s place of residence as required by State regulations and provide written documentation of all such visits.
    2. Conduct and document competency demonstrations of all PCA providers at least annually for tasks the staff member is required to perform.
    3. Adhere to agency infection control and safety procedures by performing and documenting nursing observations, procedures, and interventions in the home according to the established care plan in a safe and competent manner.
    4. Effectively communicate and interact with clients/caregivers and families in a supportive manner.
