Position Summary

The RN Case Manager is responsible for providing skilled nursing care, coordinating patient services, and overseeing the hospice plan of care in collaboration with the interdisciplinary team. This role ensures patients and families receive compassionate, high-quality end-of-life care that promotes dignity, comfort, and support.

Key Responsibilities
  • Assess patient and family needs to develop, implement, and update individualized care plans.
  • Provide direct nursing care, including symptom management, wound care, and medication administration.
  • Act as primary point of contact for patients, families, and caregivers regarding clinical concerns and care coordination.
  • Collaborate with physicians, social workers, chaplains, CNAs, and volunteers to ensure holistic patient care.
  • Educate patients and families on disease process, medications, and end-of-life care.
  • Monitor patient condition, promptly report changes, and adjust care plans as necessary.
  • Document all care and communications in accordance with regulatory and organizational standards.
  • Participate in interdisciplinary team meetings and contribute to care planning.
  • Provide on‑call nursing support as assigned.
  • Ensure compliance with state, federal, and organizational policies and standards.
Qualifications
  • Current, unencumbered Registered Nurse (RN) license in [state].
  • Minimum of 1 year nursing experience (hospice, home health, or critical care preferred).
  • Strong clinical assessment, communication, and organizational skills.
  • Ability to work independently and manage multiple patients and priorities.
  • Compassionate, patient-focused, and committed to excellence in end-of-life care.
  • Valid driver’s license, reliable transportation, and proof of insurance.
