Job Overview
Vaiden Community Living Center is looking for a MDS Coordinator to join our growing team. The role requires a highly skilled, caring, compassionate professional to manage resident MDS data and care plans.
Responsibilities- Maintain and update schedules of MDSs that are due
- Coordinate the work of the Interdisciplinary Team to complete the MDS, including CAAs and care plan
- Enter or supervise input of MDS into computer systems
- Act as the leader of the MDS and care plan meetings
- Coordinate the completion of the care plan
- Other tasks as required
