What You’ll Do & Bring

As a Behavioral Health Case Manager
, you'll play a critical role in connecting patients with the resources, services, and support they need to achieve improved health outcomes and overall well-being.

Key Responsibilities:

  • Conduct assessments, care planning, and ongoing case management services
  • Coordinate health and human services to meet individual patient needs
  • Utilize evidence-based assessment tools such as CANS and ANSA
  • Monitor patient progress and support treatment planning
  • Serve as a liaison between patients, providers, and community resources
  • Support quality initiatives aligned with CCBHC, HEDIS, ACO, and other programs
  • Assist with care coordination and patient advocacy efforts
  • Provide support in Behavioral Health Assessment Center operations as needed

  • Ability to manage multiple priorities in a fast-paced environment
  • Strong communication, organizational, and interpersonal skills
  • Collaborative, patient-centered approach to care
  • Commitment to improving behavioral health outcomes
  • Ability to work effectively with diverse populations
  • Knowledge of crisis intervention principles and behavioral health best practices
  • Strong advocacy skills and attention to detail

Required Qualifications:

  • Ability to obtain Texas HHS Community Health Worker (CHW) certification within 12 months of hire
  • Ability to complete the Texas Community Partner Program within 12 months of hire
  • Ability to obtain QMHP-CS certification within 2-3 months of hire
  • Bachelor's degree in a qualifying behavioral health-related field, Registered Nurse (RN), or Licensed Practitioner of the Healing Arts (LPHA)

Preferred:

  • Background in social work
  • Experience with risk assessments, crisis management, and de-escalation techniques
