Memorial Hermann Health System is seeking a Registered Nurse (RN) Case Manager in Pearland, Texas.
Job SummaryProvide patient care coordination, case management, and discharge planning to improve quality, patient satisfaction, and cost‑effectiveness. Collaborate with physicians, interdisciplinary teams, and payors to manage caseload and lead performance improvement initiatives.
Key Responsibilities- Coordinate patient care progression throughout the continuum.
- Work collaboratively with physicians, nurses, and interdisciplinary team members to ensure timely, appropriate care.
- Identify and resolve delays and obstacles to discharge; seek appropriate consultation.
- Apply clinical appropriateness criteria for admissions and continued stays; monitor LOS and resource utilization.
- Provide comprehensive discharge planning, including teaching, coordination, and documentation.
- Facilitate referrals to home health, hospice, medical equipment, and supplies through the Resource Center.
- Compile and analyze data (LOS, cost, readmissions, etc.) to drive continuous improvement and report performance indicators.
- Assist in the development of clinical pathways and case‑management tools.
- Act as preceptor/mentor for new hires and support ongoing education and competency.
- Ensure safety, compliance with policies, and promote professional growth.
- Perform additional duties as assigned.
- Graduate of an accredited professional nursing school;
Bachelor of Nursing preferred, or graduate of an accredited master's program in Social Work. - Valid Registered Nurse license in Texas, or Licensed Master Social Worker (LMSW) in Texas (LCSW preferred).
- Certification in Case Management within two years of hire.
- Minimum 3 years of nursing or social work experience in an acute hospital setting; equivalently experienced in ambulatory surgery, infusion/dialysis, FQHC, skilled nursing, or wound clinic.
- Experience in utilization management, case management, discharge planning, or related cost/quality programs.
- Excellent interpersonal communication, negotiation, leadership, analytical, data management, and PC skills.
- Working knowledge of discharge planning, utilization management, performance improvement, disease management, and managed care reimbursement.
- Understanding of pre‑acute and post‑acute care, community resources, physician office routines, motivational interviewing, and change management.
- Strong organizational and time‑management skills; capable of prioritizing multiple tasks.
- Ability to work independently and exercise sound judgment with physicians, payors, and patients.
