Interim Health Care, one of the nation's leading providers of home care, is now seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join our hospice care team. As a Hospice CNA, you will play a crucial role in delivering high-quality, personalized end-of-life care to patients in their homes, ensuring their comfort, dignity, and overall well-being during this sensitive phase of life.

Responsibilities
  • Patient Care:
    Provide hands-on assistance with activities of daily living (ADLs), including bathing, dressing, grooming, and monitoring vital signs.
  • Patient Care:
    Document any changes in the patient's condition and report concerns to the registered nurse or hospice team.
  • Medication Administration:
    Assist in administering medications as per prescribed treatment plans and report any adverse reactions or side effects.
  • Medication Administration:
    Maintain accurate records of medication administration.
  • Comfort Care:
    Maintain a clean, safe, and comfortable environment, making necessary adjustments to ensure patient well-being.
  • Comfort Care:
    Implement appropriate pain management techniques as outlined in the patient's care plan.
  • Documentation:
    Maintain detailed patient records, documenting care provided, observations, and changes in the patient's condition.
  • Documentation:
    Ensure compliance with all documentation and reporting requirements as per hospice regulations.
  • Collaboration:

    Work collaboratively with the interdisciplinary team, including nurses, social workers, chaplains, and volunteers, to deliver holistic care to patients.
  • Collaboration:

    Participate actively in team meetings and contribute to care planning discussions.
Qualifications
  • Licensed CNA in the State of Texas - required
  • 1+ years' experience - preferred
  • Experience with Hospice or Home Health - preferred
  • Effective written and verbal communication
  • Ability to travel - must have reliable transportation
  • Clean background check and drug screen
