Thrive Rehabilitation of Pearland is seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to join our team in Pearland, Texas. As a leading provider of post-acute rehabilitation and in-patient skilled nursing, we are dedicated to delivering high-quality care to our patients. Our serene and active setting fosters an environment conducive to healing, and our staff is trained and educated to provide exceptional care to medically complex patients.
Responsibilities:- Provide direct patient care, including vital signs, medication administration, wound care, and adaptive equipment training
- Develop and implement individualized care plans in collaboration with other healthcare professionals
- Monitor and report patient progress, identifying areas for improvement and adjusting care plans as necessary
- Collaborate with rehabilitation professionals to promote patient independence and achieve treatment goals
- Maintain accurate and detailed records of patient care, treatment, and progress
- Current LVN licensure in the State of Texas
- High school diploma or equivalent required; associate's degree in nursing or related field preferred
- Ability to work effectively in a fast-paced environment, prioritizing multiple tasks and responsibilities
- Strong communication and interpersonal skills, with the ability to work collaboratively with patients, families, and healthcare team members
- Ability to lift, move, and transfer patients, as well as stand and walk for extended periods
