Timbercreek Rehab and Healthcare Center invites Licensed Practical Nurses to deliver efficient and effective nursing care, ensuring positive clinical outcomes and resident/family satisfaction in adherence to accepted standards of practice, state and federal regulations, and licensing requirements. The role operates within the scope of the state Nurse Practice Act and includes resident care, assignment of staff, mentorship, education of nursing personnel, and collaboration with physicians and other medical professionals.

• Provide direct nursing care to residents during assigned shifts.
• Assign and supervise nursing staff assignments.
• Mentor and educate nursing personnel.
• Collaborate with physicians and other medical professionals to coordinate care.
• Maintain documentation and ensure compliance with regulatory standards.

• Active, licensed LPN in the state of practice in good standing.
• Experience as an LPN in a long‑term care, skilled nursing facility, assisted living, or memory care setting preferred.
• Strong communication skills, both written and oral.
• Professional appearance and demeanor.
• Enthusiasm for working with seniors and a flexible, energetic personality.
