- Monitor vital signs and report changes in resident behavior to the nursing and medical team
- Assist residents with daily care needs while maintaining comfort and safety
- Routinely make rounds and provide responsive resident care and services
- Support resident independence and well-being through patient-centered care
- Maintain accurate communication with nursing staff regarding resident needs
- Ensure reliable attendance and punctuality for assigned shifts
- Follow facility policies, procedures, and long-term care standards
- Perform additional duties as assigned by management in a resident-centered care environment
- Current Illinois CNA certification required
- Graduate of an accredited CNA program
- Caring, dependable, and team-oriented attitude
- Passionate about providing personalized care to residents
- Strong communication and teamwork skills
- Dependable with reliable attendance and professionalism
- Motivated to grow professionally within long-term care
