** RESPONSIBILITIES
* ** Adhere to residence policies and procedures.
* Maintains confidentiality of resident and residence information.
* Attends and participates in staff meetings, in-services, quality improvement activities and other meetings as requested.
* Assumes duties and responsibilities as assigned.
* Notifies physician of any issues related to the plan of care,  resident health concerns & medication concerns.
* Documents as necessary in the client chart.
* Facilitates ongoing contact with the resident physician or care provider as needed.
* Assists with following up with employee regarding health records and ensures compliance with immunization requirements.
* Supervises  HHA & LPNs
* Completes annual evaluations.
* Institutes disciplinary actions as needed.
* Reviews & Follows up on resident grievances, incidents and accidents.
* Participates in quality assurance meetings.
* Assists in resident referral to outside agencies, services as indicated.
* Other related duties as assigned.
** EDUCATIONAL REQUIREMENTS
*** Graduate from a school of Nursing.
* Current, valid New York State License and registration as a Registered Nurse.
* Experience in a long term care or community based setting or home care agency, preferred.
