Location: Westlake Nursing and Rehabilitation Center
Position: Licensed Practical Nurse (LPN)
Shift: 7:00 AM – 3:00 PM | 3:00 PM – 11:00 PM | 11:00 PM – 7:00 AM (Weekend and holiday availability may be required)
- Administer medications, treatments, and therapies as prescribed.
- Monitor and assess residents, reporting changes in condition to the RN and physician.
- Supervise and support Certified Nursing Assistants (CNAs).
- Maintain accurate and timely documentation in the electronic medical record (EMR).
- Assist with admissions, transfers, and discharges.
- Collaborate with physicians, therapists, social services, and other interdisciplinary team members.
- Ensure compliance with CMS, state, OSHA, HIPAA, and facility policies.
- Promote resident dignity, safety, and quality of life.
- Respond promptly to resident needs and emergency situations.
- Participate in care planning meetings, in-service education, and quality improvement initiatives.
- Current, active Licensed Practical Nurse (LPN) license in the State of Florida.
- Graduate of an accredited Practical Nursing program.
- Current CPR/BLS certification.
- Minimum of One-Year Long-term care or skilled nursing experience required.
- Strong communication, organizational, and critical thinking skills.
- Ability to work independently and as part of a multidisciplinary team.
- Experience with electronic medical records (EMR) is required.
