We recognize the importance of balancing professional and personal commitments and offer established schedule options designed to support both employees and the people who live in our residential homes.
Nursing schedules are assigned based on program needs and are intended to remain consistent from week to week. Positions may include a combination of weekday, weekend, and extended-day shifts to ensure nursing coverage during key morning and evening support times across our residential homes.
All positions include a regular weekend morning commitment and participation in a rotating holiday coverage schedule to ensure continuity of healthcare support.
Key Responsibilities- Coordinate healthcare supports within residential homes.
- Conduct health assessments and follow-up evaluations after hospital visits, urgent care visits, falls, or other health-related events.
- Maintain accurate medical, medication, and healthcare documentation.
- Collaborate with healthcare providers, pharmacies, specialists, families, and interdisciplinary team members.
- Support and advocate for people during healthcare appointments.
- Develop, implement, and monitor individualized healthcare plans.
- Assist with transitions of care, including hospital admissions and discharges.
- Ensure compliance with medication administration requirements, healthcare regulations, and agency policies.
- Provide health-related education and support to residential staff.
- Participate in quality improvement and healthcare compliance activities.
- Coordinate and facilitate MAP trainings and recertifications.
- Maintain MAP Trainer Certification and support medication administration training initiatives.
- Current Massachusetts RN or LPN license in good standing.
- Valid driver's license.
- Strong assessment, communication, and organizational skills.
- Ability to work independently while collaborating effectively as part of a multidisciplinary team.
- Proficiency with electronic documentation and Microsoft Office applications.
- Three or more years of nursing experience.
- Experience supporting people with intellectual and developmental disabilities.
- Experience in community-based nursing, residential supports, long-term care, or related healthcare settings.
