This position supports patients in Penn Hills and surrounding areas and is eligible for a $10,000 first‑year success bonus to reward the accomplishment of key achievements during your first year.

Responsibilities:

• Collaborate with the care team to manage and deliver patient care plans
• Maintain accurate charting and meet critical deadlines for documentation
• Provide direct patient care while traveling to patient locations; approximately 20% of your day will be spent driving
• Utilize tablets, mobile phones, and EMR software to support patient care and documentation

Qualifications:

• LPN license in the state where you work
• Graduate from an approved school of practical nursing
• Two years of LPN experience
• Current driver’s license and ability to travel to/from patient locations
• Proven ability to self‑manage time and schedule
• Experience with tablets, mobile phones, and EMR software
