Assignment DurationThis is a 13-week contract position.
ScheduleFull Time, 5x8s - Day/Night shift variable, requires scheduling every weekend and on holidays.
Job DescriptionThe RN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on their assigned unit. The individual must possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to their age-specific needs, specifically focusing on the adult and geriatric patient group.
Responsibilities- Collect data relevant to residents' physical and psychosocial conditions on an ongoing basis and communicate findings to the Supervisor and other members of the Interdisciplinary Team.
- Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of resident care reflecting nursing diagnoses and interventions to achieve desired resident outcomes.
- Recommend revisions to care plans after evaluating resident outcomes and indicate the need for referrals to ensure comprehensive resident care.
- Collaborate with residents/families and members of the Interdisciplinary Team, assuring customer satisfaction within the scope of nursing practice.
- Maintain current knowledge and competency in Nursing Practice and participate in the mentoring/orientation of staff.
- Provide leadership in the professional practice setting under the direction of the Nurse Manager and demonstrate knowledge and competency.
- Current NY LPN Licensure
- BLS (AHA or RQI only)
- At least 1 year of experience as an RN, with a focus on Long Term Care (LTC) experience.
