LPN Part Time Nights
Under the direction of licensed nursing personnel, performs direct and indirect designated resident care within the scope of practice as defined by certification. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served. Shows knowledge of the principles of growth and development of the life span. Possesses the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to age-specific needs, and to provide the care needed by adult and geriatric patient groups.
Meets residents' ADL needs as outlined in the plan of care and in accordance with policies and procedures, including hygiene, grooming, dressing, incontinence/catheter care, elimination/toileting needs, turning/positioning, and skin care. Completes documentation per protocol. Maintains residents' environments and safety as outlined in the plan of care and in accordance with policies and procedures. Responds promptly to call lights and alarms.
Provides safety measures such as safety mats and low beds. Uses restraints under the direction of nursing staff. Maintains cleanliness of the unit and engages in behavior and fall prevention. Uses and monitors for malfunctioning equipment appropriately. Completes documentation per protocol. Meets residents' nutritional needs as outlined in the plan of care and in accordance with policies and procedures, including meal delivery, tray preparation, and adaptive equipment.
Honors residents' preferences and choices. Completes documentation per protocol. Meets residents' rehabilitative needs as outlined in the plan of care and in accordance with policies and procedures. Engages in activities such as range of motion, ambulation, transfers, proper body alignment, and use of assistive devices. Completes documentation per protocol. Reports residents' rights, refusals, and concerns to Charge and/or Supervising Nurse in a timely manner in accordance with policies and procedures.
Identifies and promptly reports changes in residents' physical, emotional, and psychosocial status to Charge Nurse. Engages in required tasks that involve potential contact with blood, body fluids, or tissues. Demonstrates one year of experience in LTC or CNA capacity.
