Under the direction of licensed nursing personnel, performs direct and indirect designated resident care within the scope of practice as defined by certification. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served. Possesses the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to age-specific needs, providing the care needed by adult and geriatric patient groups.
Resident's ADL needs are met as outlined in the plan of care and in accordance with policies and procedures. Includes hygiene, grooming, dressing, incontinence/catheter care, elimination/toileting needs, turning/positioning, and skin care. Documentation is completed per protocol. Resident's environment and safety are maintained as outlined in the plan of care. Includes answering call lights and alarms promptly, provision of safety mats and low beds, and maintaining cleanliness of the unit.
Behavior and fall prevention measures are taken under the direction of nursing, including appropriate use of equipment. Resident's nutritional needs are met as outlined in the plan of care, including meal delivery, tray preparation, and feeding. Resident's preferences and choices shall be honored. Documentation is completed per protocol. Resident's rehabilitative needs are met as outlined in the plan of care.
Includes range of motion, ambulation, transfers, proper body alignment, and use of assistive devices. Documentation is completed per protocol. Resident's rights, refusals, and concerns are reported to Charge and/or Supervising Nurse in a timely manner. Changes in resident physical, emotional, and psychosocial status are identified and reported promptly. Occupational exposure to blood borne pathogens is reasonably anticipated. Required tasks involve potential contact with blood, body fluids, or tissues.
Requires one year of experience in LTC or as a CNA.
