Position: LPN / LVN - LTC

LPN Part Time

Under the direction of licensed nursing personnel, performs direct and indirect designated resident care within the scope of practice as defined by certification. The individual must demonstrate knowledge of the principles of growth and development across the lifespan. Possesses the ability to assess data reflective of the patient's status and interpret information as needed to identify each patient's requirements relative to age-specific needs.

Provides care needed by adult and geriatric patient groups. Resident's Activities of Daily Living (ADLs) needs are met as outlined in the plan of care and in accordance with policies and procedures. This includes hygiene, grooming, dressing, incontinence/catheter care, elimination/toileting needs, turning/positioning, and skin care. Documentation is completed per protocol. Maintains the resident's environment and safety as outlined in the plan of care and according to policies and procedures.

This includes answering call lights and alarms promptly and maintaining cleanliness of the unit. Monitors for malfunctioning equipment. Documentation is completed per protocol. Residents' nutritional needs are met as outlined in the plan of care and in accordance with policies and procedures. This includes meal delivery, tray preparation, adaptive equipment, and feeding. Residents' preferences and choices shall be honored. Documentation is completed per protocol.

Meets residents' rehabilitative needs as outlined in the plan of care and in accordance with policies and procedures. This includes range of motion, ambulation, transfers, and use of assistive devices. Documentation is completed per protocol. Reports residents' rights, refusals, and concerns to the Charge Nurse or Supervising Nurse in a timely manner. Identifies and reports changes in residents' physical, emotional, and psychosocial status promptly.

Occupational exposure to blood borne pathogens is reasonably anticipated. Required tasks involve potential contact with blood, body fluids, or tissues.
