Part Time Nights: 7:00p - 7:30a
Every other weekend required
Holiday coverage required
2 x 12.0-hour shifts per week (24 hours)
Must be able to begin orientation on a Tuesday or Thursday
Active NY LPN Licensure
BLS certification (AHA or RQI specifically)
Minimum 1 year of experience as an LPN
High School Diploma or GED
Under the direction of licensed nursing personnel, performs direct and indirect designated resident care within the scope of practice as defined by the certification
Must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit
Must demonstrate knowledge of the principles of growth and development of the life span
Must possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs
Provides care needed by adult and geriatric patient groups
Resident ADL needs are met as outlined in the plan of care and in accordance with policies and procedures within a safe and comfortable environment
Includes hygiene, grooming, dressing, incontinence/catheter care, elimination/toileting needs, turning/positioning, skin care, etc.

Resident environment and safety maintained as outlined in the plan of care and in accordance with policies and procedures
Includes answering call lights and alarms promptly, provision of safety mats and low beds, restraint use under the direction of Nursing, maintaining cleanliness of unit, behavior and fall prevention, appropriate use of and monitoring for malfunctioning equipment, etc.

Resident nutritional needs are met as outlined in the plan of care and in accordance with policies and procedures
Includes meal delivery, tray preparation, adaptive equipment, feeding, nourishments, etc.
Resident preferences and choices shall be honored

Resident rehabilitative (restorative and maintenance) needs are met as outlined in the plan of care and in accordance with policies and procedures
Includes range of motion, ambulation, transfers, proper body alignment, and use of assistive devices

Resident rights, refusals, and concerns are reported to the Charge and/or Supervising Nurse in a timely fashion in accordance with policies and procedures
Changes in resident physical, emotional, and psychosocial status are identified and reported to the Charge Nurse promptly
Occupational exposure to blood borne pathogens is reasonably anticipated
Required tasks involve potential contact with blood, body fluids, or tissues
Minimum one (1) year of experience in a long-term care (LTC) or CNA capacity — required
