LPN – Long Term Care
Schedule:
3:00 PM - 11:30 PM, 5 days per week, every other weekend, with flexible end date.
Assignment Details:
- Contract Length: 27 weeks
- Guaranteed
Hours:
40 hours per week
Requirements:
- NY LPN Licensure
- BLS (AHA or RQI only)
- 1 year of experience as an LPN in a long-term care facility
Roles & Responsibilities:
- Provide direct and indirect skilled nursing care for assigned residents.
- Collect data relevant to residents' physical and psychosocial conditions and communicate findings to the RN and the interdisciplinary team.
- Contribute to the development of personalized care plans and collaborate with residents and families.
- Recommend revisions to care plans based on evaluations of resident outcomes.
- Participate in mentoring and orientation of staff while maintaining knowledge of current nursing practices.
- Ensure a safe and healthy environment for residents, considering safety, cost, and impact in care delivery.
Compliance / Notes:
Occupational exposure to blood borne pathogens is anticipated. Required tasks may involve contact with blood, body fluids, or tissues.
