LPN – Long-term Care
Full-time, day/night variable shift, every weekend (40 hours)
Contract Length: 27 weeks
Requirements:
- NY LPN Licensure
- BLS Certification
- 1 year of experience as an LPN in a long-term care facility
Roles & Responsibilities:
- Provide direct and indirect skilled nursing care to assigned residents under the supervision of a Registered Nurse.
- Collect, assess, and communicate data relevant to residents' physical and psychosocial conditions to the interdisciplinary team.
- Contribute to the development and revision of resident care plans based on evaluations and outcomes.
- Collaborate with residents, families, and the healthcare team to ensure comprehensive care and customer satisfaction.
- Maintain current knowledge and competency in nursing practice through continued education and participation in in-service programs.
