Under the direction of the licensed nursing personnel, performs direct and indirect designated resident care within the scope of practice as defined by the certification. Must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on the assigned unit. Individual must demonstrate the ability to assess data reflecting the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to age-specific needs.
Provides care needed for the adult and geriatric patient group. Resident's activities of daily living (ADLs) needs are met as outlined in the plan of care and in accordance with policies and procedures within a safe and comfortable environment. This includes hygiene, grooming, dressing, incontinence/catheter care, elimination/toileting needs, turning/positioning, skin care, etc. Documentation is completed per protocol. Maintains resident's environment and safety as outlined in the plan of care and in accordance with policies and procedures.
This includes answering call lights and alarms promptly, provision of safety mats, low beds, and restraint use under the direction of Nursing. Maintains cleanliness of unit, behavior and fall prevention, and appropriate use of and monitoring for malfunctioning equipment. Documentation is completed per protocol. Meets resident's nutritional needs as outlined in the plan of care and in accordance with policies and procedures.
This includes meal delivery, tray preparation, adaptive equipment, feeding, and honoring resident's preferences and choices. Documentation is completed per protocol. Addresses resident's rehabilitative (restorative and maintenance) needs as outlined in the plan of care and in accordance with policies and procedures. This includes range of motion, ambulation, transfers, proper body alignment, and use of assistive devices. Documentation is completed per protocol.
Reports resident's rights, refusals, and concerns to Charge and/or Supervising Nurse in a timely fashion in accordance with policies and procedures. Identifies and reports changes in resident physical, emotional, and psychosocial status to Charge Nurse promptly. Occupational exposure to blood-borne pathogens is reasonably anticipated. Required tasks involve potential contact with blood, body fluids, or tissues.
