The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must demonstrate knowledge of the principles of growth and development across the lifespan. They must possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to age-specific needs.
Direct and indirect skilled nursing care will be provided for assigned residents. Defined policies and procedures will be followed under the supervision of a Registered Nurse. A collaborative relationship with other nursing staff, physicians, and ancillary staff is essential. An appropriate level of knowledge and clinical expertise will be maintained through continuing education and in-service programs. The LPN must have the ability to provide care to geriatric residents.
Duties include collecting data relevant to the resident's physical and psychosocial condition on an ongoing basis and communicating findings to the RN and other members of the Interdisciplinary Team. Contributing to the creation of a developmentally age-appropriate and culturally relevant plan of resident care will involve nursing diagnosis and interventions to achieve desired resident outcomes. The LPN will engage the resident/family and healthcare team in this process while promoting a healthy and safe environment.
Recommendations for revisions to the plan of care will be made after evaluating resident outcomes and indicating the need for referrals to ensure all aspects of the resident's care are addressed. Collaboration with the resident/family and members of the Interdisciplinary Team will occur within the scope of nursing practice to assure customer satisfaction. Current knowledge and competency in Nursing Practice must be maintained.
Participation in the mentoring/orientation of staff is expected. Factors related to safety, cost, and impact will be considered in planning and delivery of care. Under the direction of the RN, leadership in the professional practice setting will be provided. Demonstration of knowledge and competency is required. Occupational exposure to blood borne pathogens is reasonably anticipated. Required tasks involve potential contact with blood, body fluids, or tissues.
