Key Responsibilities of the LPN

• Collect data relevant to the resident's physical and psychosocial condition and communicate findings to the RN and Interdisciplinary Team.
• Contribute to developing a culturally relevant plan of resident care with nursing diagnosis and interventions.
• Recommend revisions to care plans based on evaluation of resident outcomes and need for further referrals.
• Maintain current knowledge and competency in nursing practice; participate in mentoring and staff orientation.
• Provide leadership in the professional practice setting under RN direction, ensuring safety and cost-effective care delivery.

Requirements of the LPN

• NY LPN Licensure
• BLS Certification
• 1 year experience as an LPN in a Long-Term Care Facility
