Seven Healthcare are seeking a dedicated Licensed Practical Nurse (LPN) for a full-time travel assignment in Penn Yan, NY. This role focuses on providing skilled nursing care for adult and geriatric patients. The LPN will work under the supervision of a Registered Nurse within a multidisciplinary team environment, emphasizing growth and development principles across the lifespan.

Key Responsibilities of the LPN Full Time (40 hours)

• Collects data relevant to resident's physical and psychosocial condition on an ongoing basis and communicates findings to RN and other team members.
• Contributes to the creation of a developmentally and culturally relevant care plan involving the resident/family and healthcare team.
• Recommends revisions to care plans based on resident outcomes and collaborates on referrals.
• Maintains current knowledge and competency in nursing practice and participates in mentoring/orientation of staff.
• Considers factors related to safety, cost, and impact in planning and delivery of care.

Requirements of the LPN Full Time (40 hours)

• NY LPN Licensure
• BLS Certification (AHA or RQI only)
• 1 year of experience as an LPN in a long-term care facility
