This role involves providing both direct and indirect skilled nursing care under the supervision of a Registered Nurse. Key responsibilities of the LPN/LVN include:

• Provide direct and indirect skilled nursing care for assigned residents
• Collect data on resident's physical and psychosocial condition, and communicate findings
• Contribute to creating a care plan that is age-appropriate and culturally relevant
• Recommend care plan revisions after evaluating outcomes and collaborate with team
• Maintain nursing practice knowledge and competency, and mentor staff as needed
• Ensure safety, cost-effectiveness, and coordinate care delivery under RN supervision

Requirements of the LPN/LVN include:

• NY LPN Licensure
• BLS Certification
• 1 year of experience as an LPN in a long-term care facility
