Under the direction of licensed nursing personnel, performs direct and indirect designated resident care within the scope of practice as defined by certification. Must demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Ability to assess data reflective of the patient's status and interpret necessary information to identify each patient's requirements relative to age-specific needs.
Provides care needed by adult and geriatric patient group. Resident's Activities of Daily Living (ADLs) are met as outlined in the plan of care and in accordance with policies and procedures. Ensures residents are assisted with hygiene, grooming, dressing, incontinence/catheter care, elimination/toileting needs, turning/positioning, and skin care. Documentation is completed per protocol. Maintains resident's environment and safety as outlined in the plan of care and in accordance with policies and procedures.
Answers call lights and alarms promptly. Provides safety mats and low beds. Uses restraints under the direction of Nursing. Maintains cleanliness of unit and implements behavior and fall prevention strategies. Monitors and appropriately uses equipment to prevent malfunctions. Documentation is completed per protocol. Addresses resident's nutritional needs as outlined in the plan of care and in accordance with policies and procedures.
Includes meal delivery, tray preparation, adaptive equipment use, feeding, and nourishment provision. Honors resident's preferences and choices. Documentation is completed per protocol. Addresses resident's rehabilitative needs following the plan of care and in accordance with policies and procedures. Involves range of motion, ambulation, transfers, proper body alignment, and use of assistive devices. Documentation is completed per protocol. Reports resident's rights, refusals, and concerns to Charge Nurse or Supervising Nurse in a timely manner in accordance with policies and procedures.
Identifies and promptly reports changes in resident physical, emotional, and psychosocial status to Charge Nurse. Occupational exposure to blood borne pathogens is reasonably anticipated. Required tasks involve potential contact with blood, body fluids, or tissues. One year of experience in Long-Term Care (LTC) or Certified Nursing Assistant (CNA) capacity is required.
