Key responsibilities of the LPN include:

• Collect data relevant to resident's physical and psychosocial condition and communicate findings to RN and healthcare team.
• Contribute to the creation and revision of culturally relevant care plans that achieve desired resident outcomes.
• Maintain knowledge and competency in nursing practice through continuing education.
• Provide direct and indirect skilled nursing care under RN supervision.
• Consider factors related to safety, cost, and impact in care delivery.

Requirements of the LPN include:

• Current NY LPN Licensure
• BLS Certification (through AHA or RQI)
• 1 year of experience as an LPN in a long-term care facility
