The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must demonstrate knowledge of the principles of growth and development across the lifespan. The LPN must possess the ability to assess data reflective of the patient's status. The LPN interprets the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs.

The LPN provides the care needed by the adult and geriatric patient group. The LPN provides direct and indirect skilled nursing care for assigned residents. The LPN follows defined policies and procedures under the supervision of a Registered Nurse. The LPN maintains a collaborative relationship with other nursing staff, physicians, and ancillary staff. The LPN maintains an appropriate level of knowledge and clinical expertise through continuing education and inservice programs.

The LPN has the ability to provide care to geriatric residents.

The LPN collects data relevant to the resident's physical and psychosocial condition on an ongoing basis. The LPN communicates findings to the Registered Nurse and other members of the Interdisciplinary Team. The LPN contributes to the creation of a developmentally age-appropriate and culturally relevant plan of resident care. The LPN reflects nursing diagnosis and interventions to achieve desired resident outcomes. The LPN promotes involvement of the resident/family and healthcare team in the care process while ensuring a healthy, safe environment.

The LPN recommends revisions to the plan of care after evaluating resident outcomes. The LPN indicates the need for referrals to ensure all aspects of the resident's care have been addressed. The LPN collaborates with the resident/family and members of the Interdisciplinary Team. The LPN assures customer satisfaction within the scope of nursing practice. The LPN maintains current knowledge and competency in Nursing Practice.

The LPN participates in mentoring and orientation of staff. The LPN considers factors related to safety, cost, and impact in planning and delivering care. Under the direction of the Registered Nurse, the LPN provides leadership in the professional practice setting. The LPN demonstrates knowledge and competency. Occupational exposure to blood borne pathogens is reasonably anticipated. Required tasks involve potential contact with blood, body fluids, or tissues.
