LPN – Long Term Care

Schedule:

Full Time Nights, 40 hours per week, with weekend and holiday requirements.

Assignment Details:
Contract duration is 23 weeks.

Requirements:

• New York LPN Licensure
• BLS certification (from AHA or RQI)
• At least 1 year of experience as an LPN

Responsibilities:

• Provide direct and indirect skilled nursing care for assigned residents.
• Collect and communicate data relevant to residents' physical and psychosocial conditions.
• Collaborate with the interdisciplinary team to create and adjust individualized care plans.
• Participate in the mentoring and orientation of staff.
• Maintain competency through continuing education and in-service programs.
• Ensure safety and quality of care in a professional practice setting.
