Schedule:

Part Time Nights (2 shifts of 8 hours or 3 shifts of 8 hours), every other week.

Hours:

24 hours per week Contract Weeks: 19

• New York LPN Licensure
• BLS Certification (AHA or RQI)
• 1 year experience as an LPN
• High School Diploma or GED

• Provide direct and indirect resident care as per the established plan of care.
• Assist residents with activities of daily living (ADLs) in a safe and comfortable environment.
• Maintain resident safety and a clean environment according to policies and procedures.
• Ensure the nutritional and rehabilitative needs of residents are met per the plan of care.
• Document all resident care activities as required.
• Report any changes in resident condition to supervising nursing staff promptly.
