The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on the assigned unit. The individual must demonstrate knowledge of the principles of growth and development across the lifespan and possess the ability to assess data reflective of the patient's status. They must interpret the appropriate information needed to identify each patient's requirements relative to age-specific needs.

The LPN provides direct and indirect skilled nursing care for assigned residents. The LPN follows defined policies and procedures under the supervision of a Registered Nurse. They maintain a collaborative relationship with other nursing staff, physicians, and ancillary staff. The LPN is responsible for maintaining an appropriate level of knowledge and clinical expertise through continuing education and in-service programs. The LPN shall have the ability to provide care to geriatric residents.

Job Duties include collecting data relevant to residents' physical and psychosocial conditions on an ongoing basis. The LPN communicates findings to the RN and other members of the Interdisciplinary Team. The LPN contributes to the development of a culturally relevant plan of resident care reflecting nursing diagnosis and interventions to achieve desired outcomes. They involve residents and families in this process while promoting a healthy, safe environment.

The LPN recommends revisions to the plan of care after evaluating resident outcomes. They indicate the need for referrals to ensure all aspects of the resident's care are addressed. The LPN collaborates with residents, families, and members of the Interdisciplinary Team within the scope of nursing practice to assure customer satisfaction. The LPN maintains current knowledge and competency in Nursing Practice.

They participate in the mentoring or orientation of staff. The LPN considers factors related to safety, cost, and impact in planning the delivery of care. Under the direction of the RN, the LPN provides leadership in the professional practice setting. They demonstrate knowledge and competency. Occupational exposure to blood borne pathogens is reasonably anticipated. Required tasks involve potential contact with blood, body fluids, or tissues.
