Licensed Practical Nurse (LPN)

This is a 13-week contract position.

24 hours per 4 weeks with 2 weekend shifts required. Contracts must run through Feb/March, end date flexible, starting on a Tuesday or Thursday.

The Licensed Practical Nurse (LPN) is responsible for providing direct and indirect skilled nursing care for assigned residents in a manner that reflects knowledge and skill appropriate to the age and specific needs of the patient group. This role demands collaboration with nursing staff, physicians, and ancillary staff to ensure the delivery of high-quality care.

Responsibilities:

• Collect data regarding residents' physical and psychosocial conditions and communicate findings to the RN and members of the Interdisciplinary Team.
• Contribute to the development of a culturally relevant plan of resident care that reflects nursing diagnoses and interventions to achieve desired outcomes.
• Evaluate resident outcomes and recommend revisions to the care plan as needed, ensuring collaboration with residents, families, and other healthcare team members.
• Maintain competency in nursing practices and participate in the mentoring and orientation of new staff members.
• Demonstrate leadership within the professional practice setting under the direction of the RN.

Required Experience / Certifications / Licensure:

• New York LPN Licensure
• BLS Certification
• 1 year of experience serving in an LPN capacity at a Long-Term Care Facility
