Licensed Practical Nurse (LPN) - Part Time

This is a 13-week contract position.

Part Time Evenings (2x8s/3x8s), every other weekend, holiday required.

The Licensed Practical Nurse (LPN) will perform direct and indirect resident care under the direction of licensed nursing personnel. The LPN must demonstrate knowledge and skills necessary to provide care appropriate to the adult and geriatric patient population, including assessing and interpreting patient data to meet age-specific needs.

Responsibilities:

• Meet residents' Activities of Daily Living (ADLs) needs as outlined in the plan of care, including hygiene, grooming, and positioning.
• Maintain residents' safety and environment as per the plan of care, ensuring cleanliness and prompt attention to call lights.
• Address residents' nutritional needs and preferences, including meal delivery and feeding assistance.
• Support residents' rehabilitative needs in accordance with the plan of care, including mobility and body alignment.
• Report changes in residents' physical, emotional, and psychosocial status to the Charge Nurse promptly.

Required Experience / Certifications / Licensure:

• New York LPN Licensure
• BLS Certification (AHA or RQI only)
• 1 year experience as an LPN, preferably in a long-term care or CNA capacity
• High School Diploma or GED
