LPN - LTC

Part-Time LPN Position

This position requires variable shifts during the day and evening, scheduled as needed. Holiday availability is required.

Under the direction of licensed nursing personnel, the Licensed Practical Nurse (LPN) performs both direct and indirect resident care activities while adhering to the established scope of practice. The LPN must effectively communicate and collaborate with nursing staff to provide exceptional care to residents, demonstrating knowledge and skills appropriate to their age-specific needs.

• Assist with residents' activities of daily living (ADLs) including hygiene, grooming, dressing, and toileting needs.
• Maintain a safe and comfortable environment, promptly answering call lights and alarms, and enforcing safety protocols.
• Ensure nutritional needs are met as per the plan of care, including meal delivery and assisting with feeding.
• Support rehabilitative needs including range of motion exercises and proper body alignment.
• Report residents' rights, refusals, and any changes in physical, emotional, or psychosocial status to nursing staff timely.
• Comply with infection control and safety standards, anticipating exposure to blood borne pathogens.

• Active New York LPN Licensure.
• Current Basic Life Support (BLS) certification (AHA or RQI only).
• Minimum of 1 year of experience as an LPN, preferably in long-term care (LTC) or as a Certified Nursing Assistant (CNA).
• High School Diploma or GED.
