LPN - LTC

Full-Time LPN – Every Weekend

This is a 13-week contract position.

Full-time day/night variable shift scheduled as needed by the facility.

The Licensed Practical Nurse (LPN) is responsible for providing skilled nursing care to residents, focusing on the unique needs of adult and geriatric populations. The LPN must demonstrate both the knowledge and skills necessary to assess patient conditions and collaborate with the healthcare team to facilitate optimal patient outcomes.

Responsibilities:

• Collect data relevant to the resident's physical and psychosocial condition and communicate findings to the Registered Nurse (RN) and other interdisciplinary team members.
• Contribute to the development of individualized care plans based on nursing diagnoses and interventions, ensuring involvement from residents and their families.
• Evaluate resident outcomes and recommend necessary revisions to care plans, indicating any required referrals.
• Maintain current knowledge and clinical competence through ongoing education and in-service programs.
• Participate in the mentoring and orientation of new staff, ensuring a collaborative and supportive work environment.
• Demonstrate leadership within the professional practice setting under the direction of an RN.
• Manage occupational exposure to blood borne pathogens and ensure compliance with safety standards.

Required Experience / Certifications / Licensure:

• Current New York LPN Licensure.
• Basic Life Support (BLS) certification.
• A minimum of 1 year of experience as an LPN, preferably in a long-term care facility.
