Licensed Practical Nurse (LPN) - Full Time Contract

This is a 13-week contract position.

* All contracts must run into February/March, end date flexible. Must be able to start on a Tuesday or Thursday.

We are seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality care to our residents. The LPN will perform both direct and indirect nursing care under the direction of licensed nursing personnel, ensuring that the needs and preferences of each resident are met.

Responsibilities:

• Assessing and meeting residents' Activities of Daily Living (ADLs) needs as outlined in their care plan.
• Maintaining a safe and clean environment for residents, including responding promptly to call lights and alarms.
• Meeting residents' nutritional needs, ensuring meal delivery, and assisting with feeding as necessary.
• Providing rehabilitative care, including range of motion exercises, ambulation assistance, and positioning.
• Reporting changes in residents' physical, emotional, and psychosocial status to the Charge Nurse in a timely manner.

Required Experience / Certifications / Licensure:

• Valid New York LPN Licensure.
• BLS certification (AHA or RQI only).
• 1 year of experience as an LPN, preferably in long-term care.
• High School Diploma or GED is required.
